Introduction

Welcome to the GoodX Career Academy application for shadow placement. This form should only be completed after receiving your GoodX Career Academy Course Certificate of Completion.

Please have the following documents ready before starting to complete the form:
1. Proof of payment of the R200 application fee*.
2. Copy of ID document.
3. A good-looking photo of yourself - an ID photo is seldom complementary
4. Proof of residence - we must know which practice would be accessible for you.

6. Grade 12 certificate (if you are still in school, you can add your latest report card).
7. Certificate of completion of the GoodX Career Academy Phase 1 course.
8. A YouTube link to a short video in which you introduce yourself.
9. A 300-word story about any topic in your handwriting - please do not type the story. Add a photo of the handwritten story to the application form.
